When you visit Medical Cannabis Outreach for assistance with the medical card application process, one of our friendly, in-house medical marijuana doctors will provide a free assessment in order to approve or deny you for use of medical cannabis with the program. These approvals are fast, easy, and convenient, assuming that you have one of the severe or debilitating health conditions that qualifies you for use of medical marijuana and/or CBD oil. To make this process as fast and hassle-free as possible, our cannabis-friendly physicians highly recommend that you bring all necessary paperwork, including two proofs of identification, age, and residency (such as a current driver’s license, passport, or birth certificate), as well as your medical records. Then, we can move forward with the medical card consultation and evaluation!
In-House Consultation & Evaluation
Once you have been approved to apply for the Compassionate Use Of Medical Cannabis Program, our in-clinic physicians will provide a formal evaluation to ensure that you would be a good fit for the program. During your visit, we will look over your medical records to ensure that you have a qualifying health condition and that you would benefit from the use of medical marijuana and/or CBD oil. In-House Photo ID & State Fee Payment
If you decide to proceed with the medical card application, our clinicians will guide you through the process, from filling out the application form and paying the mandated state fee to having your photo ID taken. Two-year medical card registration:
- $200 for qualifying patients

Three-year medical card registration:
- $250 for qualifying patients
- $125 for patients with SSDI or U.S. veterans with DD214/discharge papers
