Telemedicine Visit in Illinois

New Patients

All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.


Existing Patients

We offer recertification for patients who because of various reasons do not have their card anymore.

Arizona Patient Medical Cannabis Card Instructions

  1. Check Eligibility
    • Review the medical condition requirements and the Medical Marijuana Program Qualifying Patient Checklist from the AZDHS website.
  2. Doctor’s Recommendation
    • Must get an Arizona physician’s recommendation.
  3. Application to ADHS
    • Visit the ADHS Portal
    • Provide:
      • Arizona ID or Proof of Residency
      • Physician Recommendation
      • Photo of Yourself
      • State Fees
  4. Mandated state fees.
    • The Arizona Department of Health Services charges $150 for a medical marijuana registry identification card. This is also the renewal fee for a medical cannabis card. The Department offers the Arizona medical cannabis card at a lower fee of $75 for patients who are currently on SNAP (Supplemental Nutrition Assistance Program).

 


Caregiver and Minor Requirements

  • If the patient is a minor (under 18)
    • Review and follow the Medical Marijuana Program Qualifying Patient under the Age of 18 Checklist from the AZDHS website.
  • If the patient is an adult using a caregiver

  • Cachexia or wasting syndrome
  • Cancer
  • Crohn’s Disease

  • Glaucoma

  • Hepatitis C
  • Human Immunodeficiency Virus (HIV)

  • Post Traumatic Stress Disorder (PTSD)

  • Severe and chronic pain
  • Severe nausea
  • Seizures, including those characteristic of epilepsy
  • Severe or persistent muscle spasms, including those characteristic of multiple sclerosis

Medical Cannabis Certification & Ongoing Treatment

Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.

Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
