All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.
Existing Patients
We offer recertification for patients who because of various reasons do not have their card anymore.
Arkansas Patient Medical Cannabis Card Instructions
- Confirm Eligibility and Get Physician Certification
- Note: You must submit this form to the state within 30 days of the physician’s signature, or you will need a new certification.
- Prepare Your Required Documents
- The ADH strongly recommends applying online through their official patient registry portal. Gather clear digital copies of these items:
- Physician Written Certification
- The Completed form from Step 1, signed by your Arkansas-licensed MD or DO.
- Proof of Identity & Residency
- A clear copy of your valid, non-expired Arkansas Driver’s License or state-issued ID (from the DMV).
- Completed Patient Application Form
- The form obtained from the ADH website. When applying online, you will fill this out digitally.
- Physician Written Certification
- The ADH strongly recommends applying online through their official patient registry portal. Gather clear digital copies of these items:
- Submit Your State Application
- The state application is submitted to the Arkansas Department of Health (ADH).
- Online Application (Recommended for faster processing):
- Go to the ADH Patient Registry System (online portal)
- Register/Create an Account
- Start a New Patient Application
- Complete the application and upload your required documents (Physician Certification and AR ID)
- Pay the non-refundable $50 state fee
- Submit your application
- Mail-In Application (Slower processing time):
- Download, print, and fill out the official forms from the ADH website.
- Include your completed forms, a copy of your AR ID, and a $50 check or money order payable to the Arkansas Department of Health (ADH).
- Mail the package to the address provided on the application form.
- Online Application (Recommended for faster processing):
- The state application is submitted to the Arkansas Department of Health (ADH).
- Approval and Accessing Your Card
- Wait for Processing
- The ADH processing time is typically 10 to 14 business days from the date they receive the complete application and payment.
- If you apply online, you can log back in to check your application status.
- Receive Your Card
- Online Applicants
- Once your status shows “ISSUED”, you can often download and print a digital copy of your card immediately from the online portal. A physical card will also be mailed to you.
- Mail-In Applicants
- You must wait for the physical card to arrive in the mail before you can purchase medical cannabis.

Caregiver and Military Restrictions
- Patients under 18 require a caregiver (parent/guardian)
- Members of the Arkansas National Guard and U.S. Military are prohibited from obtaining a card.

Medical Cannabis Certification & Ongoing Treatment
Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.
Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
