Connecticut Medical Cannabis Card

1. Complete Your Physician Certification First
   • Unlike some states, Connecticut requires your physician, APRN, or physician assistant to initiate your registration — you cannot start your own application.
   • During your certification appointment, your provider will collect your email address and phone number and enter your certification directly into the DCP system.
   • After certification, you’ll receive an email invitation to access the Connecticut DCP Patient Portal.

2. Required Documents to Upload
   • Gather these documents and scan/photograph them clearly before starting your application:
     • Proof of Identity
       • A valid government-issued photo ID (see the DCP’s accepted list for full details).
     • Proof of Connecticut Residency
     • Recent Photo (may be required depending on portal prompts)
3. Fees & Submission
   • Log in to the patient portal using your emailed invitation and complete your registration.
   • Upload all required documents through the portal.
   • There is no state registration fee for Connecticut medical marijuana patients or caregivers.
   • Submit your completed application.
4. Approval and Accessing Your Registration Certificate
   • Wait for Approval
     • Qualifying patient applications can take up to 30 business days to process.
   • Access Your Certification
     • Connecticut does not mail a physical card. Once approved, you’ll receive your permanent registration certification by email.
     • You may cut the certificate from the approval letter, present the full letter, or display it from your phone at the dispensary.
     • It’s recommended you laminate any printed copy to protect it.

Caregiver and Minor Requirements

1. If the patient is a minor (under 18)
   • A parent or legal guardian must be registered as the patient’s primary caregiver.
2. If a patient’s certification indicates a need for a caregiver
   • The Department will not issue the patient’s registration certificate until the caregiver’s application is submitted and approved. The caregiver must register before the patient’s certificate can be finalized.
