New Patients
All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.
Existing Patients
We offer recertification for patients who because of various reasons do not have their card anymore.
Oklahoma Patient Medical Cannabis Card Instructions
- Obtain a Physician Recommendation
- Unlike many states, Oklahoma does not have a restricted list of qualifying conditions. The decision is left to the recommending physician’s professional judgment.
- Note: The application you submit to the sate must be dated and signed by the physician within 30 days of your online submission to the OMMA.
- Prepare Your Required Documents
- The OMMA application is submitted entirely online. Gather and prepare clear, digital copies of the following documents:
- Proof of Identity
- Clear, digital color copy of one of the following:
- Unexpired OK driver’s License or ID
- U.S. Passport
- Certain Tribal ID Cards
- Clear, digital color copy of one of the following:
- Proof of Oklahoma Residency
- Clear, digital color copy of one of the following:
- OK Driver’s License or ID (if unexpired)
- Prior year’s OK State Tax Return
- Utility Bill from the preceding calendar month (no cell/internet bills)
- Current OK Rental Agreement/Property Deed
- Clear, digital color copy of one of the following:
- Digital Photo
- A recent, clear, color, full-face digital photograph (similar to a passport photo) taken within the last six months. It must have a plain white or off-white background.
- Physician Recommendation
- The completed, signed Adult Patient Physician Recommendation Form from Step 1.
- Reduced Fee Proof (if applicable)
- Copy of your Medicare card, SoonerCare (Medicaid) enrollment letter, or 100% Disabled Veteran status letter from the VA.
- Proof of Identity
- The OMMA application is submitted entirely online. Gather and prepare clear, digital copies of the following documents:
- Apply Online to the OMMA
- This is the final step where you create your profile and submit everything to the state.
- The OMMA MedPortal
- Go to the OMMA website (Oklahoma Medical Marijuana Authority) and access the MedPortal or licensing portal.
- Create an Account: Register a new account using a valid email address that you check regularly.
- Start Application: Log in and select the option to start a New Patient Application.
- Enter Information: Fill out your personal details, including your name, address, and physician’s information from the recommendation form.
- Upload Documents: Carefully upload the digital copies of your Proof of Identity, Proof of Residency, Digital Photo, and the Physician Recommendation Form.
- Pay the Fee:
- Standard Fee: $100+$4.30 processing fee ($104.30 total)
- Reduced Fee (with proof): $20+$2.50 processing fee ($22.50 total)
- Payment is made online via credit or debit card.
- Submit: Review your entire application for accuracy (especially dates and document clarity) and submit it.
- Approval and Receiving Your Card
- Wait for Approval
- The OMMA has up to 14 business days to review and approve or reject your application.
- They will contact you via email if your application is incomplete, giving you an opportunity to correct any errors.
- Receive Your Card
- Once approved, you will receive an approval email from the OMMA.
- Your physical Medical Marijuana Patient License Card will then be mailed to you via U.S. Postal Service. You must wait to receive the physical card before legally purchasing medical marijuana.

Medical Cannabis Certification & Ongoing Treatment
Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.
Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
