New Patients
All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.
Existing Patients
We offer recertification for patients who because of various reasons do not have their card anymore.
Massachusetts Patient Medical Marijuana Registration Instructions
- Obtain Your Physician Certification
- You must first be certified by a qualified Massachusetts healthcare provider through the Medical Use of Marijuana Program.
- After certification, you’ll receive an email with a Patient PIN Number and registration instructions.
- Create Your Account and Submit Your Application

- Use your PIN to create or claim your MassCIP account and complete the “Register as a Patient” application.

- Required Documents to Upload
- Gather these documents and scan/photograph them clearly before starting your application:
- Government-Issued Photo ID
- A current MA ID, U.S. passport, U.S. military ID, or Permanent Resident Card.
- Proof of Massachusetts Residency
- A utility bill less than 60 days old, or other accepted document matching your registration info.
- Passport-Style Photo
- Color, square, plain white/off-white background, taken within the last six months.
- Government-Issued Photo ID
- There is no longer a fee to register for a Program registration card (a $10 fee applies only to replace a lost or stolen card).
- Gather these documents and scan/photograph them clearly before starting your application:

- Wait for Approval
- A 14-day temporary registration is generated as soon as your provider submits certification, allowing dispensary access while your card is processed.
- Receive Your Card
- Once your online registration is complete and approved, a physical Program ID Card is mailed to you.

Caregiver and Minor Requirements
- If the patient is a minor (under 18)
- The minor must be certified by two physicians, one of whom must be a pediatric specialist or pediatrician.
- A Personal Caregiver (typically a parent/guardian) must also register.
- If the patient is an adult using a caregiver
- Patients may designate up to two Personal Caregivers, who must separately register with the same required documents.

Medical Cannabis Certification & Ongoing Treatment
Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.
Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
