New Patients
All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.
Existing Patients
We offer recertification for patients who because of various reasons do not have their card anymore.
Montana Patient Medical Marijuana Registry Instructions
- Create an Account and Submit Your State Application
- Montana’s Medical Marijuana Program is administered by the Cannabis Control Division within the Montana Department of Revenue (formerly DPHHS).
- TransAction Portal (TAP)
- Go to the Montana TransAction Portal
- Register as a New User
- You’ll need a valid email address.
- Start a New Registry ID Card Application and indicate whether you intend to purchase from a dispensary or grow your own.
- Note: Applications must be submitted within 60 days of your physician’s certification.
- Required Documents to Upload
- Gather these documents and scan/photograph them clearly before starting your application:
- Signed Physician’s Statement for a Debilitating Medical Condition
- Government-Issued Photo ID
- Montana driver’s license, state/tribal ID, or similar accepted document.
- Proof of Montana Residency (if not already reflected on your ID)

- Fees & Submission
- Complete the online TAP application with your contact and provider details.
- Upload all required documents.
- Pay the $20 application fee online.
- Submit your completed application.
- Approval and Accessing Your Card
- Temporary Card
- Immediately after submitting, you can print a temporary card from TAP, valid for 60 days while your application is processed.
- Permanent Card
- The state mails your permanent registry card, valid for one year, generally within 30 days of approval.

Caregiver and Minor Requirements
- If the patient is a minor (under 18)
- The minor needs certifications from two physicians.
- A parent or legal guardian must register as the minor’s designated purchaser (Montana’s term for caregiver) and must be at least 21 years old.
- If the patient is an adult using a caregiver
- The designated purchaser registers separately through TAP and must be 21 or older.

Medical Cannabis Certification & Ongoing Treatment
Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.
Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
