Telemedicine Visit in Illinois

New Patients

All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.


Existing Patients

We offer recertification for patients who because of various reasons do not have their card anymore.

West Virginia Patient Medical Cannabis Card Instructions

  1. Account Creation and Application
    • The patient must register for an account on the OMC’s official online licensing system
      • https://omc.wv.gov/patient-application/Pages/default.aspx
      • Create and complete a New Patient Registration application
  2. Required Documents (To Upload)
    • You will need to gather and upload several key documents to complete your application:
      • Physician Certification
        • The copy of the official form your certified physician provided you.
      • Proof of Identity
        • A copy of a valid government-issued photo ID (e.g., WV Driver’s License, WV State ID, or U.S. Passport).
      • Proof of WV Residency
        • Documentation showing your current West Virginia address (e.g., utility bill, voter registration card, or lease agreement).
      • Digital Photo
        • A recent, clear, passport-quality photo of your face (full face visible, no hats/sunglasses/filters).
          • This will appear on your ID Card.
  3. Application Fee
    • There is an annual $50 application fee for both initial and renewal applications.
    • Payments can be made online (often via ACH/electronic payment) within the portal.
      • Alternatively, you can mail a check or money order (payable to the WV Department of Health) but you must include your Application ID number in the memo line and mail it to the OMC office.
    • Financial Hardship Waiver
      • If your household income is below 200% of the Federal Poverty Level, you may request a fee waiver.
        • You must upload acceptable proof of income (W-2s, recent pay stubs, proof of eligibility like SNAP/Medicaid) with your application instead of paying the fee.
  4. Review, Approval, and Card Issuance
    • Once all tabs are completed, documents are uploaded, and the fee (or waiver request) is submitted, you click Submit.
    • The OMC reviews applications in the order they are received, typically within 30 days.
    • You will receive email updates regarding the status of your application
      • Rejected for Correction
        • If information or a document is missing or incorrect, the application is rejected back to you.
        • You log in, make the correction(s), and resubmit.
      • Approved
        • You will be notified via email that your application is approved.
    • Upon approval, you will log back into the portal to download and print your digital Medical Cannabis ID Card.
      • You must present this card at a licensed dispensary for purchases.
  5. Renewal
    • The WV Medical Cannabis ID Card is only valid for one year and must be renewed annually.
    • Renewal Process:
      • Obtain a new annual Physician Certification.
      • Log into your existing OMC account.
      • Complete and submit a Patient Renewal Application.
      • Pay the $50 renewal fee (or submit a fee waiver request).

Medical Cannabis Certification & Ongoing Treatment

Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.

Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
