Telemedicine Visit in Illinois

New Patients

All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.


Existing Patients

We offer recertification for patients who because of various reasons do not have their card anymore.

Missouri Patient Medical Cannabis Card Instructions

  1. Create an Account and Submit Your State Application
    • After the doctor submits the form, you must complete your registration on the state’s online portal.
    • DHSS Compliance Portal
    • Note: You must apply to the state within 30 days of the date the doctor signs on your Physician Certification Form.
  2. Required Documents to Upload
    • Gather these documents and scan/photograph them clearly before starting your application:
      • Clear Digital Photo
        • A recent, clear, color photograph of your face, similar to a passport photo (taken within the last 3 months).
      • Government-Issued Photo ID
        • A copy of your valid State or Federal Photo ID (e.g., Driver’s License or Passport).
      • Proof of Missouri Residency
        • One of the following:
          • Valid MO Driver’s License or ID
          • Current MO Motor Vehicle Registration
          • Recent (within 60 days) MO utility bill with your name/address
  3. Fees & Submission
    • Fill out the entire online application, including your contact details, and indicate if you wish to apply for a Cultivation ID Card (to grow your own medicine).
    • Upload all required documents.
    • Pay the application fee (usually $26 for the Patient ID Card, and $102 for the optional Cultivation ID Card). The DHSS only accepts online payment.
    • Submit your completed application
  4. Approval and Accessing Your Card
    • Wait for Approval
      • The DHSS has up to 30 days  to process your application.
      • Watch your email for updates. If your application is incomplete, they will notify you, and you will have 10 days to make corrections.
    • Download Your Card
      • Once approved, you will receive an email notification from the DHSS.
      • Log back into your Complia Portal account.
      • Navigate to your License Dashboard and select the option to Download License.
      • Save the digital copy and print a physical copy of your ID card for use at dispensaries.

Caregiver and Minor Requirements

  • If the patient is a minor (under 18)
    • Parent/Legal Guardian Consent Form  is required
    • Caregiver must also apply separately
  • If the patient is an adult using a caregiver
    • A Patient Authorization Form is needed

Medical Cannabis Certification & Ongoing Treatment

Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.

Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
