Telemedicine Visit in Illinois

New Patients

All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.


Existing Patients

We offer recertification for patients who because of various reasons do not have their card anymore.

California Patient Medical Cannabis Card Instructions

  1. Register with the California Medical Marijuana Identification Card (MMIC) Program
    • California’s Medical Marijuana Identification Card (MMIC) Program is administered through each county’s health department rather than a single statewide application portal.

  3. Required Documents
    • Physician’s Recommendation
    • Government Issued Photo ID
    • Proof of California Residency
  4. Mandated state fees
    • Counties have varying fees. $100 or less depending on the county.
  5. Application Review
    • County reviews and verifies your physician recommendation and supporting documents
    • Most applications are processed within 30-35 days.

 


Caregiver and Minor Requirements

  1. If the patient is a minor (under 18)
    • Minors may qualify for a Medical Marijuana Identification Card (MMIC).
    • If the minor is not emancipated or legally self-sufficient, the county health department will verify information with the parent, legal guardian, or person authorized to make medical decisions for the minor.
    • A parent or legal guardian must participate in the application process and provide any required identification or supporting documentation.
    • Emancipated or self-sufficient minors may be required to provide additional documentation.
  2. If the patient uses a Primary Caregiver
    • Apply in person with the patient at the county MMIC office
    • Present a valid government issued photo ID
    • Have their photograph taken for their MMIC
    • Complete and sign the Primary Caregiver Declaration on the application

Medical Cannabis Certification & Ongoing Treatment

Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.

Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
