Telemedicine Visit in Illinois

New Patients

All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.


Existing Patients

We offer recertification for patients who because of various reasons do not have their card anymore.

Michigan Patient Medical Marihuana Card Instructions

  1. Create an Account and Submit Your State Application
    • The Michigan Medical Marihuana Program (MMMP) is administered by LARA’s Cannabis Regulatory Agency (CRA).
    • BMMR Online Portal
      • Go to the MMMP Registration Portal
      • Create a New Patient Account
        • You’ll need a valid email address.
      • Start your application and designate your certifying physician within the system.
    • Note: Once you submit your application, your physician has 15 days to certify it online.
  2. Required Documents to Upload
    • Gather these documents and scan/photograph them clearly before starting your application:
      • Government-Issued Photo ID
        • A valid Michigan driver’s license or state ID.
      • Proof of Michigan Residency (if not reflected on your ID)
      • Recent Passport-Style Photo
  3. Fees & Submission
    • Complete the online application with your personal information.
    • Upload all required documents.
    • Pay the $40 application fee (covers a registry card valid for two years).
    • Submit your completed application for your physician’s certification.
  4. Approval and Accessing Your Card
    • Wait for Approval
      • The MMMP has up to 15 business days to approve or deny an application after your physician certifies it. If your physician certifies within 24 hours of submission, cards can sometimes issue within 2–3 business days.
    • Download or Receive Your Card
      • Once approved, you can check your status by logging into your account at michigan.gov/BMMRonline.
      • Your physical registry card will be mailed; note that an application/certification alone is not valid for legal possession — you must have the issued card and a valid photo ID.

 


Caregiver and Minor Requirements

  1. If the patient is a minor (under 18)
    • A parent or legal guardian must be designated as caregiver on the application before the minor can be certified.
  2. If the patient is an adult using a caregiver
    • The caregiver applies separately through the same LARA portal and is linked to the patient’s account.

Medical Cannabis Certification & Ongoing Treatment

Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.

Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
