New Patients
All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.
Existing Patients
We offer recertification for patients who because of various reasons do not have their card anymore.
New Jersey Patient Medicinal Cannabis Program Instructions
- Get Certified by a Registered Practitioner
- Schedule an appointment (telemedicine or in-person) with a Medical Cannabis Outreach practitioner registered with the New Jersey Medicinal Cannabis Program (MCP).
- After your evaluation, your practitioner will issue you a Registry ID Number and Reference ID Number.
- Create Your Account and Submit Your Application

- Go to the NJ MCP Portal
- Create a Username and Password using your Registry ID and Reference ID Numbers.
- Complete your registration, including selecting an Alternative Treatment Center (dispensary) where you’ll shop (you can change this anytime).

- Required Documents to Upload
- Gather these documents and scan/photograph them clearly before starting your application:
- Proof of New Jersey Residency
- A current NJ government-issued ID, or two of the following: a utility/cell phone bill from the last 90 days, or a W-2/1099/1098 or other IRS/NJ Division of Taxation correspondence from the last year (Form 1040/1095 not accepted).

- The Medicinal Cannabis Program offers free digital ID cards for new and renewing patients — there is no state registration fee.
- Gather these documents and scan/photograph them clearly before starting your application:

- The MCP reviews applications and will respond by email with next steps or confirmation.

- Once approved, your digital ID card is available directly in your patient portal account for use at dispensaries.

Caregiver and Minor Requirements
- If the patient is a minor (under 18) or an incapacitated adult
- A parent or legal guardian living at the same address typically submits the patient’s registration application on their behalf and must also register as caregiver.
- If the patient is an adult using a caregiver
- The caregiver registers separately in the portal. Caregivers who are not immediate family of the patient must submit fingerprints for a criminal background check.

Medical Cannabis Certification & Ongoing Treatment
Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.
Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
