New Patients
All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.
Existing Patients
We offer recertification for patients who because of various reasons do not have their card anymore.
New York Patient Medical Cannabis Instructions
- See an OCM-Registered Practitioner
- Schedule an appointment (telemedicine or in-person) with a healthcare practitioner registered with the New York Office of Cannabis Management (OCM) to certify medical cannabis patients.
- Automatic Registration
- If your practitioner certifies you, they enter your certification directly into the state’s Health Commerce System.
- You are automatically registered — no separate application, portal account setup, or state fee is required.
- Your certification includes your Registry ID Number.
- Access Your Certification
- Download your digital, barcoded certification from the patient portal associated with your practitioner or telehealth provider.
- Present your digital certification along with a valid government-issued photo ID at any licensed dispensary.
- Renewals
- Certifications must be renewed with your practitioner before they expire (there is no separate state renewal fee).
- A lapsed certification does not require a new “application” — a fresh certification from your practitioner automatically re-registers you.
Caregiver and Minor Requirements
- If the patient is a minor (under 18)
- A parent or legal guardian is generally required to be involved in obtaining certification and administering the patient’s medical cannabis.
- If the patient is an adult using a caregiver
- Caregivers are certified and registered through the same practitioner-driven process as patients.
Qualifying Conditions for a Medical Cannabis Certification in New York.
New York has no fixed list – practitioner discretion.
Medical Cannabis Certification & Ongoing Treatment
Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.
Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
