Telemedicine Visit in Illinois

New Patients

All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.


Existing Patients

We offer recertification for patients who because of various reasons do not have their card anymore.

Ohio Patient Medical Cannabis Card Instructions

  1. Obtain a Physician Certification
    • You must be diagnosed with a qualifying condition by a certified physician.
    • The Certification Visit
      • Find a Certified Physician: Schedule an appointment with an Ohio-licensed Doctor of Medicine (MD) or Doctor of Osteopathy (DO) who holds a Certificate to Recommend (CTR).
      • Attend Your Appointment: The doctor must establish a “bona fide” relationship with you, which includes reviewing your medical records to confirm your qualifying condition. An in-person or telemedicine visit is typically required at least once per year.
      • Physician Submits Profile: If approved, the doctor will create your profile in the Ohio Patient & Caregiver Registry and electronically submit their recommendation.
  2. Complete and Pay for State Registration
    • Once the doctor creates your profile, you will receive an email from the state to finalize your registration.
    • Access the Registry
      • Wait for the Email: You will receive an email from the Ohio Medical Marijuana Registry (check
        spam/junk folder) with the subject line “ACTION NEEDED: Ohio MMCP Registry – Account”

      • Click the Link: Click the link in the email to access your profile and create a password.
      • Review and Confirm: Log into the registry and review all personal details (name, address, etc.) the
        physician entered to ensure they are accurate and match your ID.
    • Finalize Registration & Fee
      • Accept the Legal Statement.
      • Pay the Annual Registration Fee:
        • Patient Fee: $50
        • Caregiver Fee: $25.00
        • Note: Patients qualifying for Veteran or Indigent status may be eligible for a 50% reduction in
          the state fee, which the doctor must indicate in your profile.
  3. Download and Use Your Card
    • Receive Your Card
      • Immediate Access: After you successfully submit the fee, your card is typically activated immediately.
      • Download and Print: Go to the “Card” tab in your Registry profile. You can download and save the
        digital card to your phone or computer and print a copy. Ohio does not typically mail a physical card by
        default.
    • Visit a Dispensary
      • You must present your digital or printed Registry Card and your valid Ohio Driver’s License or state-issued ID at a licensed Ohio dispensary to purchase medical cannabis.

Medical Cannabis Certification & Ongoing Treatment

Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.

Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
