All patients must have at least one of the qualifying conditions to apply for a medical certification. After verifying you meet at least one qualifying condition, ensure you have what you need for a smooth appointment process by reviewing what to have available for your telemed appointment.
Existing Patients
We offer recertification for patients who because of various reasons do not have their card anymore.
Pennsylvania Patient Medical Cannabis Card Instructions
- Register with the State (PA DOH)
- You must create a profile in the state’s official registry before seeing a doctor.
- Create Your Account
- Go to the PA Medical Marijuana Registry (The official DOH patient portal)
- Select “Patient Registration” to create a profile
- Provide your details exactly as they appear on your government-issued ID.
- Proof of Residency: You must be a legal resident of Pennsylvania. You will need a valid PA Driver’s License or state-issued ID with your current address.
- Receive Your Patient ID: After registering, the system will instantly issue a unique Patient ID Number. Keep this number handy!
- Get Certified by an Approved Physician
- You must be certified by a doctor who is registered with the PA Medical Marijuana Program.
- The Certification Visit
- Find a Registered Physician: The DOH website provides a list of approved practitioners. Schedule an appointment (telehealth is commonly accepted).
- Attend Your Appointment: The doctor will review your medical history, discuss your condition, and determine if you qualify.
- Physician Certification: If approved, the doctor will electronically submit your certification directly to the DOH registry, linking it to your Patient ID Number.
- Note: You do not need to submit the certification form yourself; the doctor handles the submission.
- Pay the State Fee and Receive Your Card
- After the doctor submits the certification, you must log back in to finish the process.
- Complete the Application & Pay
- Wait for the Email: You will receive an email from the DOH when your certification has been successfully processed and is ready for payment.
- Log back into the PA Medical Marijuana Registry.
- Navigate to the payment section and submit the State Application Fee of $50.00.
- Note: Patients participating in certain government assistance programs (like Medicaid, SNAP, WIC, CHIP) may qualify for a waived fee.
- Confirm the Payment to finalize your application.
- Card Delivery
- Once payment is confirmed, the DOH will print and mail your Medical Marijuana ID Card to the address listed on your account.
- Cards typically arrive in 7 to 10 business days.
- You must have the physical card in hand to legally purchase medical marijuana at a licensed dispensary.

Medical Cannabis Certification & Ongoing Treatment
Medical Cannabis Outreach is now providing remote telemedicine services for all our patients to continue to provide care through the COVID-19 pandemic and beyond.
Examinations for Medical Cannabis Certification completion may be done by telemedicine until further notice without requiring in-person appointments. Patients can now have their evaluation conducted in the privacy and safety of their own home or a location of their choosing.
